Paper Request Forms are printed out by the Fileholder and handed to Writers. There is an example of the form below.
The form shows instructions on how to submit a letter of recommendation by electronic upload. It also has instructions on how to submit a paper letter of recommendation by mail.
The most important component of the paper request form is the unique document ID. You will need to provide that ID if you decide to submit your letter to Interfolio by electronic upload. If you are mailing a paper letter to Interfolio, we ask that you include the request form in the envelope. The document ID comes with a bar code that we are able to scan as soon as your letter reaches our office. This allows us to process the letter quickly and accurately.
